What should I document?

Reviewed by Dr. Elizabeth Long, Ph.D. — Director of Special Education, 32 years in public schools.

Keep copies of:

  • IEPs and amendments
  • Evaluations and assessments
  • Progress reports
  • Meeting notices
  • Parent input
  • Emails and written requests
  • Notice of Proposal/Refusal to Take Action (Prior Written Notice)
  • Behavior data
  • Service logs or progress information
  • Outside evaluations, therapy notes, observations
  • Meeting notes
  • Important dates and timelines

Good documentation isn't about building a case against the school. It helps everyone work from the same information.
